
Club Reports Submission
Please complete this form to report your club’s activities and accomplishments, Your responses help the district assess accomplishments of key performance indicators
Submission deadline: April 15, 2026
(Covering Period: July 1, 2025 to April 15, 2026)
1. From the District website https://rotarydistrict3800.org, download the editable Word file at Resources > Club Reports. From the “Download Resources”, click on “Offline Copy of the Form” to download a copy.
2. Club President to accomplish the Offline Form in collaboration with club members.
3. Once completed, Club sends via email the completed Offline Form to their Assistant Governor (AG) for his/her review.
4. Senior Assistant Governor (SAG) to oversee timely submissions within their zone.
5. If there are review points called out by the AG that need to be addressed, Club President to address them on the Offline Form.
6. AG to provide their sign-off to the Offline Form by replying with an acknowledgment email once Offline Form is satisfactorily completed.
NOTE: It is important that there is a proof/screenshot of the sign-off email from the AG. This is a mandatory requirement during the Online submission of the club’s accomplishment for the reporting period to indicate that ‘a review’ was performed.
